If You've Been Referred

Step 1: Wait for a call from my office: If a medical doctor has sent a referral, you will be contacted approximately 3-4 weeks before your appointment. If you have any questions, call 905-451-1710 ext. 25737.

N.B. if there has been an error in your doctor’s referral to our office, or if you do not meet criteria of Dr. Handelman’s practice, your referral will be refused, but it may take up to 1-3 months for your doctor to be notified by Dr. Handelman’s office.

Step 2: Please print off two copies of the following form, and have a parent complete one form, and a teacher complete the other. Either fax these forms (905-796-4158), or bring them with you on the day of your appointment.

Step 3: Print off and complete the background history form. Either fax this form, or bring it with you on the day of your appointment.

Step 4: Please bring old report cards with you on the day of the appointment.

Step 5: Please bring copies of old Psychological Testing, if this applies to your child.
